Primary tuberculous liver abscess misdiagnosed as pyogenic liver abscess associated with choledocholithiasis

Abstract

|
Although hepatic involvement is often seen in patients with miliary tuberculosis, tuberculosis of the liver manifesting as liver abscess is rare. Patients with tuberculous liver abscess tend to show poor response to antituberculous medication and often require surgery for cure. We report a case of primary tuberculosis of the liver misdiagnosed as pyogenic liver abscess associated with choledocholithiasis in an 81-year-old female. Liver biopsy demonstrated marked lymphocytic infiltration and granuloma with Langerhans cells in liver parenchyma. Tissue polymerase chain reaction for Mycobacterium tuberculosis was positive. After nine months of antituberculosis medication, tubeculous liver abscess has resolved.
